Carroll Athletic Physicals for 2016-2017 School Year

It is once again time for students to obtain their athletic physical for the 2016-2017 school year.

Physicians, Nurses, and Physical Therapists from All-Star Orthopaedics, as well as other local physicians, will be working together to give your child the best physical assessment possible. Physicals will take place at the All-Star Orthopaedics offices. The cost of the physical is $25.00, with all the proceeds benefiting Carroll ISD Athletics.

If your child is interested in participating in athletics, cheerleading or drill team next year, CISD requires that they have a current physical. For it to be current for the next school year, the physical must be dated after May 1, 2016 and before Aug. 31, 2016. Physicals dated outside of this time frame will not be accepted.

This service is provided for your convenience; however, you may obtain your physical from any Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners, or a Doctor of Chiropractic. Examination forms signed by any other health care practitioner will not be accepted. Please print the UIL Pre-Participation Physical Examination form and use this form when obtaining a physical for your child.

Tickets are being sold for specific 30 minute time frames. All tickets will be purchased online through the CISD Athletics website. Tickets went on sale March 21 and cost $25.00. There will not be refunds if you cannot make your appointment.

WEDNESDAY, MAY 18   5 p.m. – 7 p.m.
SATURDAY, MAY 21  9 a.m.- 12 p.m.

Every athlete grades 7-12 must also complete all the UIL required forms to be eligible to participate. This Sports Participation Packet and the Athletic Participation Release must be completed online.  The link to the packet and release is below.  You must know your child’s Student ID number (not social) in order to fill out the online forms.  Upon successful submission of the online forms, you will receive a confirmation email that your forms have been completed.
